Your First Visit

Thank you for choosing Vascular Care of Texas for your medical care. This page is dedicated to helping patients that are new to our office become familiar with what to expect on their initial visit.

 

Appointments

We schedule patients by appointment. If you are unable to keep your scheduled appointment, we ask that you please contact us at least 24 hours before your scheduled appointment time. This gives us the opportunity to offer your time to another patient in need.

Registration

Please download and complete the registration forms and bring these with you to your initial visit with us.

Insurance/Financial

We accept most major health insurance plans. Please contact our office to confirm that we accept yours. Please be prepared to pay any co-payments, deductibles or non-covered services at the time of your visit.

For your convenience, we accept checks, credit and debit cards as well as cash. We also accept Care Credit.

Please remember to bring your insurance card(s) as well as an identification card (driver’s license or other state-issued ID) with you so that we may make a photocopy for our records.

Medical Records

If you have any records that you feel the doctor needs to view at the time of your visit, please bring them with you or have your other healthcare provider(s) fax them to us at (972) 426-9899.

Prescriptions

Please bring a list of any medications that you are taking, including any over-the-counter medications.
